How does a Hospital / Physician know I have my Advance Care Plan on my Smartphone?

A Wallet Card is available from our website at: www.myhealthcarewishes.com/wallets.html. There is also a link in Preferences to our Website to access the Wallet Card. In the near future, health care professionals will look on the Smartphone for this App.

Where should MyHCWishes APP be located on the Smartphone?

The App should be placed on one of the 4-5 screens easily visible and NOT located within a sub directory.

How secure is my personal information?

You are assured of complete confidentiality because the data resides only the iPhone, yours, your proxy’s or a loved ones. The data does not reside on any server or cloud service.

How Do I Add/Update My Information?

You can add or update information on any display at any time by tapping the Edit button in the top right corner of the screen. To save information press the same button in the top right corner that now reads Save. When this is completed, you will be prompted to press OK.

How Do I Add Additional Persons To MyHCWishes?

Click the button Add Another Person on the bottom of the Dashboard Menu to add additional persons and their Advance Care Planning information and documents onto your device. In this way you will have your own and your loved ones’ information with you at all times.

How Many Additional Persons Can I Add To MyHCWishes?

You can add unlimited persons to the MyHCWishes App.

How Do I Look Up Information?

Once content has been added you can click on any section and view the information that will appear same as when the content was keyed in. However if you would like to see a report of all information you have saved click on the button in the top right corner of screen

How Do I Grab Contact Information?

When entering contact and address information for any contacts, proxies, physicians and other entities you can click Grab From Contacts to import the information from your Contacts library on your iPhone. It will populate the screen with whatever information is available from your Contact list. If other information is needed not included in your iPhone contacts, add or edit the record. Then press the Save butto

How Do I Get Documents Into The MyHCWishes App?

There are two ways to get documents into MyHCWishes. You can import PDF/Word Documents from your PC/MAC via email or by going to iTunes on your desktop.

How Do I Import PDF/Word Documents Via Email?

1. You must have an email address associated with your iPhone. (Note: Your iPhone email may be the same as your desktop/laptop email.)
2. Draft and send an email to the iPhone address and attach your document(s)
3. Make sure your documents are not too large to email
4. If document is larger than 1.5 MB, you may want to optimize (shrink) the document to make it smaller using Adobe Acrobat. This will not reduce the quality of the PDF/Word document.
5. Open email and click on a PDF/Word document image (this may take a little time). Sometimes you may have to repeat this step if the document is not already downloaded.
6. The PDF/Word document will now be viewable on your iPhone
7. Click the button in the top right corner of the screen
8. Click on Open In…
9. Choose and click on MyHCWishes and the document should appear
10. Click the Save button in the top right corner and choose the person to which to documents apply
11. When a person is selected, a will appear
12. Press the Save button in the top right corner to save the document in an individual’s folder
13. When prompted, press OK (Note: If using iPod, you must go to FAQ How Do I Import PDF/Word Documents Via The iTunes Library?)

What If My PDF/Word Document Is Already In My iPhone Library?

Go to the location on your iPhone where the documents reside (i.e. iBooks, Pages, DropBox, PDF Viewer), click on the document and email it back to your iPhone. Then continue to step 5 in FAQ How Do I Import PDF/Word Documents Via Email?

How Do I Import PDF/Word Documents Via The iTunes Library?

You can import one or multiple documents from iTunes to MyHCWishes at any time, regardless of the size of the files. It is easier to use iTunes Import function if you have multiple large files.
1. Connect your device (iPhone or iPod) to your PC or MAC
2. Open iTunes on your PC/MAC
3. Click on your iPhone/iPod device under DEVICES on the left side of iTunes menu
4. Click Apps button on top of page to the right of Summary
5. To assure you are on the correct page Sync Apps will appear
6. Look down this page and go to the File Sharing Section
7. Under Apps in File Sharing scroll down and double click on MyHCWishes icon
8. Click the Add… button at the bottom of MyHCWishes Documents
9. After clicking this button a Finder/Directory window will open
10. Search and locate document(s) you wish to add and press Open
11. The document(s) will copy in your device’s Document folder
12. Then go to MyHCWishes App and on the Dashboard Menu click on the Advance Care Plan Documents Section
13.When on the Advance Care Plan Document screen, click the blue button + at the top right corner
14. A screen will open with the header Document List in iTunes, which includes a list of PDF/Word documents.
15. Select the documents you wish to save and add to your iPhone, then press Save
16. Document will be saved under the Advance Care Plan Documents Section

How Do I Rename My Documents In MyHCWishes?

Go to your Advance Care Plan Documents Section where the PDF/Word documents are stored. Click on any document and then click on Rename bottom left corner and enter the new name of the document. When renamed press OK. You may wish to add a date along with document name in the description field. If a document is replacing/updating an older one, it is recommended that you delete the out-of-date document.

How Do I Email Or Text (Export) The Summary Reports to Someone? (Excluding PDF/Word documents)

1. From the Dashboard Menu with the 4 large square boxes, press the button in the top right corner of screen
2. A PDF document appears showing the summary report of ALL the information entered
3. Press the button again in top right corner
4. Two (2) options will appear: Email or Text Message
5. To Email – click on Email option and enter an email address
6. To Text – click on Text Message option and enter a mobile number (when texting the message may be broken up into several text messages due to the length of the record)
7. You can also go to any of the 3 individual sections Health Care Wishes Overview, Medical Emergency Information or Health Insurance & Specialties and Email/Text message any one of these.

How Do I Email My PDF/Word Documents To Someone?

1. Go to the Dashboard Menu with the 4 large square boxes and choose the Advance Care Plan Documents box
2. Choose which document you wish to email, click on it (Note: You can only send one document at a time)
3. When viewing the document, you will notice the Email option at the bottom right of the screen
4. Click on the Email button, and enter the email address
5. The document will be sent in a PDF format as an attachment

Will I Retain All My Information when I get a New Update?

YES, all information from previous version will be included in new updated version.

How Do I Upgrade To The Newest Version of MyHCWishes?

When an update is available, it will appear in the Updates Section of the iTunes App Store.

Can I Use DropBox?

No, MyHCWishes does not support DropBox. The DropBox feature is currently being evaluated and will be available on future versions.

How do I Delete a Person’s Record?

Slide your finger across the persons name and click on the Delete Button

I Deleted My Health Care Wishes App. Do I Need To Purchase It Again?

It is not necessary to purchase a new version. Go to the iTunes store and then to your App page and click on Install. Since you previously purchased the App under the same user account, you can then download the App free of charge. You will have to reenter the information you previously entered.

What if I want to leave a field blank and it prompts me to ‘Please Enter _____’ ?

Just add any wording in that field like ‘none’ or ‘empty’.

How Do I Know If I Have Completed Adding Information?

On the Dashboard menu there are four boxes related to four sections. For three boxes, Health Care Wishes Overview, Medical Emergency Information, and Health Insurance & Specialties, you will see a red x if incomplete or a green if completed. In the Advance Care Documents box you will see a small circle showing the number of PDF/Word documents stored. Also, for each display and section, you will see a red x if incomplete or a green if completed.

Yes, either PDF or Word file format. Many Advance Care Planning documents require signatures and notarization and therefore a PDF image of the hard copy/paper is preferable to authenticate your wishes.

How Do I Convert My Hard Copy (Paper) Documents To PDF On A Scanner?

If you own a scanner: Place the hard copy document on the scanner attached to your PC/MAC, scan into PDF format and name the document to save. You may want to add a date along with document name in the description field. When saved on your desktop or a file folder, you can then email the document or go to iTunes and import the document(s) into MyHCWishes. It is recommended that you delete any out-of-date documents. If you Do Not own a scanner: Contact a friend, a family member or your estate planner and ask them to scan the documents into PDF format for you. Send them a hardcopy to scan/digitize and they can email them directly to your iPhone email address.

How Large Can The PDF/Word File Be To Import?

It is recommended that a file be no larger than 1.5 MB.

How Do I Print A Copy of MyHCWishes Information?

Should I Add Password Protection to MyHCWishes?

This is entirely your personal decision. MyHCWishes makes critical documents accessible in any medical situation or emergency. This option is provided for those who want to secure this information regardless of the need for availability.

How Do I Access The App If I Forgot My Password?

You Can’t. There is a warning message “Warning – Passwords cannot be retrieved if forgotten!” Users will not be able to access the application if your password is forgotten. MyHCWishes App does not store any of your personal information. It is recommended you send copies of all your documentation to your home email address for backup. What is the difference between POA (Power of Attorney) and HCPOA (Health Care Power of Attorney)?

An HCPOA (Health Care Power of Attorney) gives the attorney in fact the power to make Medical decisions for (but not handle the financial affairs) of the grantor. 7.Once you clicked the Download Button the attached file will render in the compatible 3rd party Application in most cases it is Adobe Reader. There are other 3rd party PDF software packages available such as “KingSoftOffice” which will do the same thing.
8. Now go back to MyHCWishes App, go to the Dashboard Menu with the 4 large square boxes, go to your Advance Care Plan Documents Section where the PDF/Word documents are stored and click on the ‘Add’ Button top of screen to add the downloaded document.
9. The Add button will go to the file section on your Android where the PDF/Word documents reside, which in most cases is the “Download’ directory. In the Downloads Directory Click on the document you wish to add and it will be added to your document section in MyHCWishes App.
NOTES:
- If you want to just View the file and do not want to download the file in Download Folder you may do this by clicking the Preview/ or View Buttons.
- For those more familiar you can go to your widgets and add the Downloads widget to your Android screen. This way all you PDF/WORD documents for all users will be readily available for importing into the MyHCWishes App from this widget.

How Do I Email or View a PDF/WORD Document?

How Do I Email, Text or Transfer via Bluetooth (Export) The SUMMARY Report to Someone?

From the Dashboard Menu with the 4 large square boxes, press the button in the top right corner of screen. A window appears offering four options – Email, Text Message, View & Bluetooth.
To Email – click on Email option and enter an email address.
To Text – click on Text Message option and enter a mobile number (when texting the message may be broken up into several text messages due to the length of the record).
To Bluetooth (Transfer)– click on Bluetooth icon, and the Android will search for Devices, once device is found it may ask to Pair devices with a Numerical Code. On e done it will transfer the file to your desktop/laptop computer.

Note – You can also go to any of the 3 individual sections Health Care Wishes Overview, Medical Emergency Information or Health Insurance & Specialties and email/text message any one of these using the same process.
